Founded in 2001, our agency was built on a deep belief that high-quality health care doesn’t end at the hospital or skilled nursing facility door. We bring skilled nursing, physical therapy, occupational therapy, speech therapy, medical social work, and home health aide services directly to patients across the Bay Area.
Our story began with a mission. When Stanford Hospital closed its home health department, our founder, Lorna Beukema, a clinical manager there for over 15 years, chose to continue that legacy of excellence in the home setting. Alongside seven of her former Stanford colleagues, she launched Nursing & Rehab At Home to serve patients recovering from illness, surgery, or injury with the same high standards of care they'd received in the hospital.
Over 20 years later, we remain a trusted partner to hospitals, skilled nursing facilities, physician practices, and families—delivering personalized, Medicare-certified care to patients throughout the region.
